To prepare enrollment certificates, the Office of the Registrar requires the following information:
- Student name
- Mailing address
- Academic terms that need to be verified
You may submit the information above in one of the following ways:
- Download and print an enrollment certification form (PDF 25 KB requires Adobe® Acrobat® Reader)
